Try to keep your mouse steady. The less you jerk it around the more comfortable and confident you'll be with aiming in general. Try to predict your enemy's movements - line up a shot and let them move (walk, jump, or whatever) into your crosshair.
If you're uncomfortable with your sensitivity, don't be afraid to change it - but don't fall into the trap of changing it everytime you have a bad day and start missing shots.
You may see it as a cheap tactic, but spawncamping can help your aim out a lot! Just try to line up your shots as quickly as possible without losing accuracy. You'll get faster and faster naturally as you get more confident in your ability to aim.
After you have your aim down you just need to work on map awareness a bit... after that consistency in aim and reaction time is really all that separates good players from great ones in this game.

1. when standing on 2 block high wall, shoot> crouch> move to the other side because they'll think you will pop up on the same place you crouched>aim at head then shoot and start it again.
Mess around with your sensitivity see what works best. Don't spam shots the rec is moderate. If you can't aim dont crouch spam. Don't strafe the same pattern. Practice. Remeber you move slow when you are scoping.
Topo said it quite well; consistently good shots are your goal... everyone has those days where every shot is a perfect headshot, but if you can be that good whenever you play, you will become almost-a-topo.
Pinpoception (n.) - Situation characterized by hiding behind a wall as enemy shoots at you on pinpoint; they expect you to stay in the same spot, so you move, but experienced players know you will move, so you don't move, and *teehee* moment of enemy confusion will allow you to kill 'em.
